Owners and admins choose whether each practitioner sees every client or only their own. Each note can be read by all staff, by staff in the author's discipline, or by its author alone, and the author sets the default.

Schedulers book, they don't chart
Schedulers can book any client. They can't open clinical records.
Encrypted at rest
AES-256 through AWS KMS, and SOAP notes are encrypted again in the app before they are stored.
