Templates standardize your clinical documentation. Stillpoint ships with SOAP and DAP templates and lets you create your own. Templates are practice-wide, so everyone on the team uses the same structure for the same kind of session.
Templates are available on Solo and above.
Built-in templates
- SOAP Note: Subjective, Objective, Assessment, Plan.
- DAP Note: Data, Assessment, Plan.
- Free-form: a blank note with no sections.
When you first open the Note Templates page, Stillpoint also seeds practice-type-specific templates (massage progress note, therapy intake, naturopath consult, etc.) so you have a useful starting point.
Create a custom template
- Open Note Templates.
- Click New Template.
- Give it a name (e.g., "Initial Intake," "Discharge Summary").
- Define the sections you want:
- Add section headings.
- Optional placeholder prompts inside each section.
- Drag to reorder sections.
- Optionally add an AI Scribe prompt that tells the model how to shape AI-generated notes for this template.
- Click Save Template.
Use a template
When you create a new clinical note, you're prompted to pick a template. The template pre-fills the structure so you can focus on the content instead of the format.
Manage templates
- Edit any custom template from the templates list.
- Duplicate a template to spin up a variation without starting over.
- Delete templates you no longer use. Existing notes that used the template are unaffected.
Tips
- Make a template per session type (initial consult, follow-up, discharge) rather than one giant catch-all.
- Add prompts inside sections to remind yourself what to capture.
- If you use AI Scribe, the per-template AI prompt is the lever that makes generated notes match your house style.
