Clinical Notes

Stillpoint provides a secure, encrypted note-taking system designed for wellness practitioners. All clinical notes are encrypted with AES-256 encryption at rest, ensuring your clients' sensitive health information stays protected.

Creating a Note

1. Open the client's profile and navigate to the Notes tab
2. Click New Note
3. Select a template (e.g., SOAP, free-form, or a custom template)
4. Fill in your session observations, treatment details, and plan
5. Click Save to encrypt and store the note

SOAP Notes

The SOAP format is built in and includes sections for:

• Subjective -- What the client reports (symptoms, concerns, goals)
• Objective -- Your observations and measurements
• Assessment -- Your clinical assessment of the session
• Plan -- Next steps, recommendations, and follow-up

Managing Notes

• Edit a note by clicking on it from the client's notes list
• Search across all notes using the search bar
• Notes are timestamped and linked to specific appointments when applicable

Security

• All notes are encrypted with AES-256 before being stored
• Only practitioners with access to your practice can view notes
• Notes cannot be permanently deleted without admin confirmation
• Encryption keys are managed securely and never stored alongside note data

Tips

• Use templates to speed up documentation and maintain consistency
• Write notes promptly after sessions for the most accurate records
